Name Change Procedures

Adults

The petitioner must file the following documents along with their Application for Change of Name (Adult):

  1. Completed and sworn petition (petition must be typed or printed legibly).
  2. Criminal history check (Virginia State Police Form 167). You will need to submit your request directly to the State Police for them to conduct your criminal history. They will return their report to you and you must present this report when you file your Petition.
  3. Proof of previous name change(s) (i.e. marriage license, other name change orders, etc.) to initiate case.
  4. The filing fee is $42, payable by cash, check, money order or credit card.

Minor

The petitioner must file the following documents along with their Application for Change of Name (Minor):

  1. Completed and sworn petition (petition must be typed or printed legibly).
  2. Both the mother and father must sign the petition. No exception is made in the event "father" is not listed on the birth certificate.
  3. Birth certificate must be filed.
  4. Proof of previous name change(s) (i.e. name change orders, etc.).
  5. The filing fee is $42, payable by cash, check, money order or credit card.

Adult and minor petitioners must be a resident of the County or City to be allowed to file in this Court.
